For a physical card:

Permanent residents of Ohio and those who own property in Ohio are eligible for a free library card.  Only one card is allowed per individual in the Clevnet system, with the exceptions of the Books2Go library card issued to youth aged 17 and under and Teacher Cards. Madison Public Library honors the library cards of other CLEVNET libraries for the circulation of materials. There is a $1.00 charge for replacement of a lost card.  There is no charge to replace a damaged card.

To apply for a library card, adults aged 18 or older must fill out an application and present valid identification and current proof of address if address is not listed on the identification. Acceptable forms of identification include a driver’s license, state ID card, passport, and US Military ID. Acceptable proof of a current address is either a printed personal check showing the address on the application, a utility bill, or other piece of mail (dated within last 30 days) postmark received at the address shown on the application. A birthdate and signature accepting financial responsibility for borrowed materials are required for a library card.

You can access your account on the internet to renew books and other checkouts, place holds and do research.

Go to “My Account” on our website.  You will need your library card number and PIN
(Your PIN is the last four numbers of your phone number)

You can download and/or stream books, movies, magazines, music and more.  Click on eServices for more info.

You can access numerous databases for research or study by clicking on eServices – Databases.

  • 21 days*: adult, YA, tween and children’s books, CDs, magazines, recorded books, puzzles, most video games, and DVD and Blu-ray sets*
  • 14 days*: new books and equipment
  • 7 days*: DVDs and Blu-rays
  • 3 days: New DVDs, Blu-rays and video games (No renewals.)

*Items automatically will renew up to five times unless another patron places a hold on the item.

Materials borrowed from other libraries circulate under the loan, renewal and fine policies of the owning library.

Madison Public Library respects the policies of the owning library.

No Overdue Fines on Children or Young Adult Books.

Fines for All Blu-Rays, DVDs and Video Games, Equipment, and Reference materials are $1 per day if overdue.

Fines for All other materials except children’s books 10 cents per day if overdue.

Overdue Fines will accumulate to a maximum of $3.00 on any item.  Patrons with fines and fees over $10.00 will be blocked from checking out additional materials.

Patrons will be charged fees up to the cost of all unreturned or damaged materials.

Yes, the library has computers for public use in the adult and youth areas. Computer use is restricted to two hours per patron per day. We offer free WiFi access for patrons who bring in a laptop or other device. WiFi also can be accessed in our parking lot or from outdoor benches.

We offer hot and cold laminating in the Create Space. Please call ahead to make an appointment to ensure a staff person will be available to assist. 440-428-2189.

Our fax service is free. Please inquire at the Information Desk upon entering the building.

We also have a self-serve copier/printer/scanner in the Public Services area.

The same fees apply for copying. Scanning is self-serve and free.

Most items will automatically renew up to five times unless another patron places a hold. To inquire, call us or go to “My Account” on this website with your card number and PIN, and renew the items yourself. Your PIN is usually the last four numbers of your phone number.

Ohio Privacy laws limit who can pick up library holds. You may only pick up for another person if:

1) You are the parent or custodial guardian of a minor child.

2) You have the other person’s library card to present at pick-up.

Yes! Free notary services are available at the library. 

Bring the following with you: a valid, government-issued photo ID, the documents to be notarized (filled out but unsigned), and any provided instructions.

  • All Signers must be present
  • Notary publics at the library reserve the right to decline service

The “eServices” link on our website gives complete instructions for downloading books, music and movies from each service. All library apps ( Hoopla, Freegal and Libby) are free. Once you have created a CLEVNET account, you can search for titles in our catalog.

The state of Ohio and the federal IRS provide limited forms that are available at the library during tax season.
We also can print most forms from the internet for $0.10 per page.

Children may get a library card when they can sign their names.  A parent or legal guardian must sign for responsibility for a card issued to a minor.  A parent also must sign to give a child permission to check out DVDs and computer games.

Books2Go cards are available to anyone younger than 18 who does not have a parent or guardian present. These cards allow the minor to check out two books at a time.

Net Notices are email or text reminders when it’s time to return items you have borrowed.

We accept donations of books, magazines and audio-visual materials for our Friends book sales.

Patrons can make financial donations to the library via cash, check or credit card (if over $5).

Yes. If you are 18 or older and have a Madison Public Library card in good standing and a valid state ID or driver’s license you can check out a hotspot for two weeks. You also must sign a hotspot agreement. You must wait 24 hours after returning a hotspot to check out another, because they cannot be renewed. Only one hotspot can be checked out per household at a time. Hotspots cannot be placed on hold, so they are first come, first served. Read more here.
